What ICD & CPT Code would you use for Toe Pain

What code to use if New F pt came in complaining of lt great toe pain, incised, drained, and cleaned area around nail?

For toe pain I would use the ICD-9 diagnosis code 681.11.

For treatment I would use the 10060 CPT procedure code with a "TA" CPT modifier.

The modifier to use with the E/M code if used is the "25" CPT modifier. In the case where the "25" modifier is used with the E/M, the "59" AND "TA" modifiers should be used with the procedure code.
