need to know the difference between CMS-1500 and the UB-04 ( CMS-1450 )

The CMS-1500 form is the health insurance claim form used for submitting claims for health care service providers. It is a standard form that non-institutional providers use such as your family doctor or specialist. Medicare and Medicaid claims (paper) must be submitted on the official CMS-1500 form with the red ink.

The CMS-1450 (UB-04) form is the claim form to submit claims for institutional providers such as hospitals.

The Centers for Medicare and Medicaid Services is the government entity which mandates these forms use.
