This site sure has helped me out alot to answering my questions. I am just getting into medical billing and coding, this is a life saver. Just wanted to say "THANK YOU".
Thanks for visiting the site - glad it's helped you.
The
CMS-1500 form is the health insurance claim form used for submitting claims for health care service providers. It is a standard form that non-institutional providers use such as your family doctor or specialist. Medicare and Medicaid claims (paper) must be submitted on the official CMS-1500 form with the red ink.
The CMS-1450 (UB-04) form is the claim form to submit claims for institutional providers such as hospitals.
The Centers for Medicare and Medicaid Services is the government entity which mandates these forms use.
Thanks!
