How to I file a Medicare Part A claim and which form do I use to file it?

by Hoori
(Dallas, Texas)

Can u please briefly let me know how do I file the claim for Medicare Part A?

Medicare Part A claims (Inpatient Hospital care) are filed using form UB04 form. I haven't done any billing for Part A but understand filling out the UB04 can be overwhelming if you haven't done it before. It looks very different from the CMS-1500.

If you're looking for a how-to guide Solutions sells a Quick and Easy Guide to Filling Out The UB04 Forms which is geared towards billers. I've found their ebooks to be very helpful.

Hope this helps some.

If any other visitors have more insight on the UB04 or filing Medicare Part A claims - Please comment by clicking the link below.

Thanks,

Gina
