CPT Code - Removal of Malignant Neoplasm of Kidney and Ureter

A patient is admitted for removal of malignant neoplasm of kidney and ureter. Under general anesthesia incision is made in the skin of the flank and carried down to muscles, fat and membrane overlying kidney.

The kidney, ureter, and bladder are mobilized. The ureter, major blood vessels and a small part of the bladder are severed and cut off, then removed through the flank incision. Controlling bleeding is sutured, catheterization and clamps are removed. A layered closure is performed.
What would be the CPT code for the surgeon to bill for the service and also the ICD-9-CM volume III procedure code for the significant procedure?
