In the example contract it states percentage that would be due from the doctor, the $500.00 monthly minimum billed is this the billing service minimum monthly charge or minimum doctor bills per month?

Laura,

In that example the $500 minimum is the minimum paid to the billing service. This is really intended to cover the costs associated with maintaining the client.

I've had situations where the cost of serving the client (our overhead) was more than we were charging for some months - and no business can survive long like that. Of course you would vary this depending on your situation.
